Error Message - ingesting via Photo Mechanic

For about a week now when I ingest images after a shoot I get an error message that 1 image did not ingest (usually about a third down the list). I am able to copy the failed image from the disk into the ingest folder manually (image is not harmed).
It seems to do it on various disks.
Causes? Is it my camera, disk, Photo Mechanic, computer or a combination?
Body: Nikon D3
Disk: Lexar 8GB 300x
Ingesting via PhotoMechanic
Computer: Mac (Leopard)

->> Any time I get this message it's because I have filled (overfilled) the card - if I change cards before its filled then ok but if I totally fill the card it seems that one image won't load - perhaps its overfilling the card - meaning it tries to write the last image but not enough room so one file is messed up.
Kirk PM...are you listening?
Not sure if my reasoning is sound but it is one thought.
