->> I worked in TV news for a good bit. (Got paid to learn how to edit video!) I can absolutely imagine how this sort of thing could happen. A news director/executive producer was most likely literally shouting at people & not-so-subtly threatening them with their jobs if they didn't get the pilot names RIGHT NOW.

The worst thing you can do in an environment like that is appear to be doing nothing. The more of a whipped-up frenzy you're in, the more you're doing *something*. So pausing, reading, and considering what you've just been told by the "NTSB" would very, very much be doing "nothing." Making a graphic and putting it on the air RIGHT NOW is doing "something."

I'm just surprised, as others have mentioned, that we don't see this type of gaffe more regularly.

WTF? One look at the names should have made someone say, 'Wait just a damn minute!'

This, to me, is one of the biggest problems with the industry today, and not just in television. Everyone is so concerned with getting a story into the ether first, with so many digital-first newsrooms, we often don't take the time to do what newspapers have always been able to do best ... find the story, uncover the back story and, most importantly, get it right!
