Where to Make Ordering Form/Envelope Combo?

| ->> I am shooting my local Little League and need to hand out price sheets/ordering forms prior to the day the pictures will be taken. I have seen these forms other photographers use that have a perforated tear off envelope at the bottom of their forms so parents can fill out the form, tear off the envelope, fold it up the order form and stick it in the envelope and enclose payment and hand in the envelopeon photo day. I need to make 700-1000 of them but I can't seem to find any place online to make what I want/need. I was wondering if anyone could point me in the right direction. Thanks in advance. |

->> Kevin who fulfills your orders? In my case the envelopes are done by MVP as part of the entire process of shooting T&I. The envelopes are OCR'ed by MVP and their computers know what each line on the envelope corresponds to in terms of product.
I guess that you could use them to do envelopes even if you don't use them to run the orders.

Anyway what I was trying to say is that for SMALL groups with 200 or less kids I just throw the order form on-line and have printed versions at the venue on photo day. For groups where we're looking at 1000+ I like the envelopes and LOVE not having to do any of the fulfillment. |

As many have said see if your lab can provide custom forms.
In my case I fullfill most of my orders, smaller leagues, and had my own order form created which includes a peel off label that I use as the mailing label. The customer fills this out which saves me a TON of time and effort. When packageing I just peel off the label from the form and place on the mailer. The name and address is also copied to the portion I keep and there is a second copy that the customer keeps.
I had a simple 8.5x11 piece of paper but had to copy the name and address to the mailer, will never go back to this.
Another option if you plan on full filling your own orders.
